RMA Policy

Whenever you need to return parts to Control Systems, Inc for any reason, you must first obtain a RMA number by submitting the RMA Request Form located below. The following information is required in order to complete your RMA request: company name, your name, original PO number if you know it, your PO number for the RMA, part number, quantity, serial number, reason for return or problem with part and delivery address. If you need the RMA rushed please indicate that in the comment section of the form. Upon acceptance and approval you will receive an E-mail notification with a RMA number and return instructions. We ask that all RMA's be shipped within 10 days of receiving your RMA number. If you are returning parts there will be a restock fee.

Please note that we request all customers not to take any credits on returned parts until Control Systems, Inc has issued a Credit Memo. When a credit is issued on your account you are welcome to use the credit on future orders or on open invoices; no check will be issued for the credit.

  1. Download and fill out request form.
  2. Email form to joyt@c-s-inc.com or fax to (402) 991-1124.
  3. Be sure to include your email address so a RMA number can be issued with shipping instructions.
